Scrip Order Form : Gift cards are available from over 300 national and local retailers. Groceries, clothing, dining out, home improvement, even gasoline can all be purchased through SCRIP. By putting your regular household shopping dollars to work, you earn money for our school without spending a single additional penny!
Extended Day Information & Registration: The St. Bernadette Extended Day Program provides afterschool care from 3:00 to 6:00 PM for Kindergarten through 8th grade. This program is separate from the Preschool Day Care Program. It serves working families of regularly enrolled St. Bernadette School students
Milk Order Form: A choice of Chocolate or White Milk, $95 per child per school year

PreK students coming 2 days per week: $45.00/year

PreK students coming 3 days per week: $60.00/year
Homework Club: Homework Club is an afterschool club providing teacher supervised homework time for students to complete homework before leaving school. The club is held every Monday - Thursday for students in grades 4-8 after school except professional days and holidays
Request a Tuition and Extended Day Receipt - In order to receive a receipt for tuition and/or extended day please submit this form. Allow up to two weeks for processing.
Change of Contact Information - If your home address, e-mail address, or any of your phone numbers have changed since the beginning of the school year please submit this form to the main office.
