Health Office Guidelines and Policies

If your student requires prescription medication:

1. Students are not permitted to carry ANY medication to school.
2. All medications should be labeled, dated and in its original pharmacy container.
3. First doses should be given at home and monitored.
4. Required Forms:
   • Doctor's orders, signed by parent and the doctor.
   • Medication Administration Plan (See Health Forms Page)
