Mrs. Sharp and Mrs. Edmands would like to welcome you to the new school year. The health

Our goal as nurses is to help your children stay healthy through parent guidance and student
teaching, in order to stay IN school to be educated. With this in mind, we ask you to complete
the daily wellness checklist located in the forms tab each morning before school. This form will
not be collected, but will serve as a reminder of precautions we are all taking. We would like our St. Bernadette School community to know we are here to offer our support
and guidance.Medication Administration
We would like to inform you of the policies that have been put in place to ensure the health and safety of children needing medications during the school day. Our school district requires that all health forms must be on file in your child's health record before we begin to give any medicine at school, including over-the-counter medications. These forms can be found on the Health Forms tab.
If your student requires prescription medication or an EpiPen please complete the Prescription Medication and EpiPen form and submit it to the health office before the start of school.
When your child needs a medication to be given during the school day, please act quickly to follow these policies so we may begin to give the medication as soon as possible. Thank you for your help.
Allergies

We have established protocols to help keep each child with Allergies safe while here at SBS. Our success depends on having classroom and cafeteria protocols and receiving appropriate medical information from each affected family. We have established safe classroom protocols at all class levels and offer an allergy free table in the cafeteria. These spaces are cleaned to eliminate any food residues. If your child has a food allergy and you do not see a problem with your child eating at the regular lunch tables, you may download, sign, and return this form allowing your child to eat with other classmates.
Sports Participation

All students partcipating in sports at St. Bernadette School must submit a copy of a physical exam done within the last 13 months to the St. Bernadette School Health Office. No student will be allowed to practice or play a sport until the Health Office has received this information and cleared him or her with the coaches to partcipate.
Body Mass Index (BMI) Screening Program

Massachusetts State law requires that all schools to conduct BMI screenings on those children in grades 1, 4 & 7 and to share those results with their parents. Once your child is screened, you will receive a letter with the results from the SBS Health Office. Below is additional information about the BMI Screening process and Nutritional information.
